Cant find numbers or name on door. door with windows on each side

Door at our church needs new Hinge I have no information on the door.lower hinge has two screws in upright jamb and two in threshold at a 45 deg angle. upper hinge has two screws in the header of the door

just looking for the correct parts. I know the top hinge is worn out the door drags on the handle side. when the handle is lifted the top hinge moves side to side.

Thanks for posting. Please see our 79-098 pivot. I think this might be what you're looking for. Let me know if the dimensions match up to your hardware. I'm afraid we only have this in aluminum. You would have to spray paint it to match your door finish.
