Take a look and compare the specifications of the 39-155 and 39-156 Caradco operators, along with the related items assocated on both product pages, to your original hardware. 

It looks like your window frame is a bit damaged too but, unfortunately, that is not something that we can help with and would be specific to the original manufacturer.
