I'm afraid we don't have a version of this operator with an arm that long. Your photos here appear to be focusing on the hinge that's mounted under the operator, though. Is that what you needed instead? If it is, then I would recommend the 36-048. The video below will also go into more detail.

A quick learner from Abington, PA says:
After I sent the email, I suddenly understood the anatomy of the crank device. Now I think they are going to be fine. I tried to order a left and right device but neither one of the them was in stock. So I signed up to be reminded when they come in. Also I am changing the ballasts on some of my double hung tilting windows. Is there anything else I should replace in addition to the ballasts? Finally what lubricant would you suggest for casement and double hung windows?

Oh, great! I'm glad you got it sorted.

In addition to the balances, you should check the condition of the top sash guides; these are nylon caps installed on the top two corners of the sash frame. Just make sure there isn't any visible damage. 

As for lubrication, it can't hurt to use silicone spray on the hinge track. I like silicone spray because it doesn't attract dirt or debris like oil tends to.
