Hello there!
Thank you for reaching out to us. Your operator looks like it may be an older version of the 39-387 and 39-388. For new brackets, see the 39-513 and 39-514. As for the hinge, check out the 36-048. Note that you'd want to replace the top and bottom hinges at the same time. Review the specifications for these parts and see if they match up with what you need for this repair.
