Casement operator 39-067

Do you have a casement operator, dimensions like 39-067 in white with offset leg going into crank, as in the attached picture?

I have found a couple of matches, but none are exact. The 39-066 has an 8-3/4" link arm, but the gear arm is different. The 39-089 has a similar gear arm, but no offset and a short link arm. In fact, I could not find any operators in this style that have a gear arm that bends down. Normally, it bends up. Could it be yours was bent after the window was installed?
