The piece that goes in the hole that the crank handle attaches two.

1 plate that goes on the window.

Thanks for posting! Regarding the first part of your request, are you referring to the worm gear on the inside of the operator that cranks the operator? If so, this is not something that can be replaced so you'll need to replace the entire mechanism.

The window operator you have here can be replaced with the 39-387 and 39-388 Split-Arm Operators. To confirm that these are the right fit, be sure to look over the dimensions and compare them.
