Damage to balance and pivot assembly on both sides

Attached pictures show damage to both balance and pivot assemblies. Just took pics of one window. Looking at the rest of the house there are multiple instances(at least six) of the balance assembly not in its proper location. It looks as if the original install did not have enough fastener engagement and the balance assembly shot down toward the pivot assembly when the fastener pulled out of the frame. Is this a known issue with these windows? We've owned the house since new and being in FL rarely open the windows. A couple that are failed have never been opened.

At any rate, will need some help identifying replacement parts.

The first part to look at is the pivot shoe, which is our 15-228. This is specifically designed to work with two sizes of coil spring: either the 3/8" thick Series 165 or the 1/2" thick Series 160

I think you have the first one, but you should measure to be sure. You will need the stamp printed on the old spring to determine the proper weight calibration anyway, so removing one of these is a necessary step before you purchase anything. 

Lastly, it looks like you have a triple spring cover, so assuming you have the smaller Series 165 then the corresponding cover would be the 15-205

As for whether this is common, I would say no. The screw pulling out of the track is usually the last thing that happens. Based on your description of the issue it just sounds like these weren't properly installed to begin with, but you should also check the screw holes in the track itself to ensure they aren't stripped or damaged in some way.
