Start by taking a look at the 79-127 pivot set used on Arch Aluminum commercial doors. This one looks similar to the one you show in your photos. The only difference might be on the lower threshold pivot. You might need to cut off the leg that attaches to the side of the door jamb, but you won't know that until you remove your existing pivot hinge. Compare the specifications of the 79-127 pivot set along with the related video.
