Your balance looks like a match for our Series 795 which comes attached with the 15-195 closed cam T-lock shoe. I recommend removing your shoe from the window to confirm this matches what you have. There should also be a stamp on the metal channel that will indicate what spring strength option you'll need. 

One important detail to note is that the Series 795 attaches to the top of the track using a screw, not a hook. If yours needs a hook then let me know; we can still help but your balance will have to be custom made.
