Double-Hung Tilt Window - Broken Pivot Shoe Identification

A quick learner from Rhode island says:
Hi,
Top window slams down and remains in an open position. Looks like the pivot shoe(?) is broken, leaving the window to fall off of the spiral tilt balance / shoe and down the channel. I was browsing and didn't seem to find a part that looks similar to this.

Thanks for any input!

Tom from SWISCO responded:

That's definitely a broken pivot shoe. We can help but I'm not sure what size you need. Can you confirm the thickness and width?

The width is probably 1-1/4" but I can't say for sure what the thickness would be. Maybe 1/2". If I'm right then you should be able to use our 15-046 no problem. Look it over and see what you think.
