I need to replace the tilt mechanism for my double hung windows.

Thanks for posting. This looks like a T-Lock Shoe used with Series 795 Channel Balances. It's tough to make out exactly which you have from these photos. See the 15-194 1" wide shoe, the 15-195 1-1/4" wide shoe, and the 15-195B 1-1/4" wide shoe with an open cam. Compare each with your old shoe and see what matches best.
