Thanks for posting! The balance you need wouldn't be the Series 795 Channel Balance since it does not have T-lock shoe. Your balance has a 65-105 Heavy Duty Pointed Hook attached to the bottom which means the replacements will need to be a custom assembly.

To determine the correct replacements for your repairs, I'll need to know the length of the metal and the stamp from the channel. I'll also need to see a photo of how the balance installs at the top of the window channel and the back of the channel to see its shape.

Regarding the pivot bar, I'm happy to identify a replacement but I'll need to see a photo of it removed from the sash.

The shoe the hook is installed in can likely be replaced with the 15-208 Pivot Lock Shoe but you'll need to be sure to compare the dimesnions.
