I think we can help with this balance, but looking at its design I don't think it's the extended-travel type. This looks like a pretty typical non-tilt channel balance.

Before we go any further, can you show me a picture of the top fitting? Your photos show the bottom, but that only tells half the story. Seeing the top fitting will allow me to narrow down the list of possibilities. 

As for your top sash guide, at first glance it looks like our 18-033, but you will need to review our dimensions to confirm.

Thanks! Based on what I see here, you have either our S351-33 or S390-33, depending on the depth of the bottom shoe. That's the only difference between them. I recommend measuring your shoe and referring to our dimensions to confirm the best match.

Question, does top sash have same balance as bottom sash?

Not necessarily. In a lot of cases, sure, the top and bottom sash will have identical balance configurations. But I've seen them be different enough times that my official recommendation is to always double check. When it comes to balances, you never want to guess.
