Looks the same as you as your Caradco 15-104 except the front face where the adjustment screw is located. My part is only 1/4" wide across the front. All the other dimensions are the same.

Just found the old one and it has the numbers 730-553 on the side.

Yes, I have seen this type and unfortunately haven't been able to find a source. I'm not sure if you can put the metal hook in the 15-104 friction shoe, either. You may be out of luck.
