Handle detaches from Truth window operator when closing window

Parts information: This window is roughly 14" wide and was installed around 1994. The handle has 40085 and 24 printed on it. The window operator has Pat.No. 4,840,075, Made in the USA, and TRUTH printed on it.

Problem: The handle and the metal shaft from the window operator slowly spin out of the rest of the operator as the handle is cranked to close the window (pictured in IMG-2672). I am able to put the handle and shaft back in the operator and use it to help close the window while someone stands outside slowly pushing on the window. The handle/operator continues to be able to crank the window open without detaching. It does not show well in the pictures but the inner gears on the operator appear to be completely intact and move smoothly. I do not know if there is a part missing or if something is broken as the handle and shaft came out the very first time we attempted to close the window. What would you recommend, do I need to replace the entire window operator? Would you provide the appropriate part number(s)?

Would you also recommend an operator cover replacement part number? The current one has a piece missing near where it meets the handle.

The arm appears to have 30318 stamped on the underside.
