Thanks for reaching out to us. Your pivot shoe reminds me of the 15-191, but you'll want to ensure the specifications align with your old one.

The compatible springs are the Series 160 (sold separately). There should be a weight code stamped on the band of the old springs. For a new double spring support, see the 15-150D.

Is there a diagram showing how the springs are attached to the shoe?

I'm afraid we don't have a diagram that shows how to install the springs to the shoe. However, you'll notice two holes at the top of the 15-191 pivot shoe. This hole is bisected by a slot that the tip of the S160 spring fits into, flat ways, then twists in the round hole to lock in place. I hope this helps.
