Thanks for reaching out to us. The balance shown here looks like a match for the Series 795. This uses a screw to install but let me know if yours uses a hook. We can still help but it would have to be a custom order.

We believe the metal channel might be 25" long but we recommend you measure to be sure. If it's indeed 25", see the 25E stamp. A pair of balances with this stamp can support a sash weighing 25 to 28 pounds.

Note that the S795 comes with the 15-195 closed cam shoe by default. If yours has an open cam, you'll want the 15-195B instead. The video below will show how to pop these shoes on and off.
