Hey, Kim. Looks like you need a pivot shoe. I'm not sure exactly what you need, but start with our 15-046. Carefully go over our dimensions. If they match up, I think this would work in place of your old shoe.
I'm not sure what the part at the top of your picture is. Were you also looking for that? If so, can you tell me its function?
