Thanks for posting! Your top sash guide reminds me of the 18-073 Top Sash Guide made by Fiberlux. Take a look over the dimensions and compare them to your guide to confirm that this is the right replacement. I am confident this should work for you repairs but you'll need to be sure to double check those measurements.
