Can someone help me with an identification for the Truth Hardware operator pictured and with how to find a replacement?

Take a look at our 39-187. Carefully look over the pictures, the dimensions, and the exact shape of the link arms to see if this matches what you need. If your link arm is a bit longer, see if the 39-067 is a better match.

Just to clarify, the 39-187 and 38-188 are the same operator, just opposite sided, correct? Also, what comes with the operators? Will I need to purchase gaskets and mounting screws separately?

Correct, the 39-188 is the opposite-handed version of the 39-187. Indeed, all secondary components such as the 39-114L Handle, the 39-513 or 39-514 Stud Brackets, or the 39-303 Gasket, are sold separately.
