Hey Mike. This looks a lot like our Series 795 to me, but that bottom T-Lock Shoe is a bit unusual. I'm forwarding this discussion to one of my senior colleagues just to make sure there's nothing off here, and I'll get back to you as soon as we can with more insights.

The manufacturer of your window balance has gone out of business. When I was replacing these balances, I would use Series 795. You would need S795-30 with a stamp option of 30C. Your bottom T-Lock shoe looks to be 1" wide, so you will also need the 15-194, sold separately. The video below shows how to install this shoe.
