Can you please identify the parts I need to fix this sliding patio door lock?

Your handle looks like an earlier version of our 82-003, which was used by WT industries. I'm not sure if you can adapt it to your sliding door, though. The differences could be too great. You might want to consider a surface mounted handle like our 82-005, instead. What do you think?

So is 82-005 a "universal" replacement? If I surface mount it, the latch would have to move... I am not sure if it will work. Are there installation instructions you can post?

We do not have detailed installation instructions, but the 82-003 just screws to the door. There isn't too much involved, aside from drilling the new installation holes in your door at the appropriate position. Are you able to remove that circular washer in the center of the frame? If you are able to remove that, I don't see why a surface-mount handle couldn't install right there.
