Looking for the correct Channel Balance Replacement
We took the following markings off the existing balance.
34-4- IU CNC 2
We do not know what the markings describe, only 34 looks like the length.
Please provide what information we need to select the proper replacement and if it requires any additional plastic parts, their part numbers

Hi Frank. We need to see pictures of the nylon attachments at either end of the balance. Also, please confirm the length of the metal channel, not including the nylon attachments. Normally for a balance with this stamp number it would be 35", not 34". But best to check to be certain. Many thanks.

Hello again! Thanks for getting back to us. This is definitely going to be either our S380-35 or S385-35. The only difference between them is the depth of the shoe, which I cannot make out from your images. If it's 1" deep, then you want our S380-35. If it's 1-3/32" deep, then you want our S385-35 instead.
Make sure to account for wear in your measurements. What that means is, your shoe's depth may be less than it was. In that case you want to use your best judgement. If it's over 1" but less than 1-3/32", odds are it was in fact 1-3/32" but wore down over time from constant friction.
I am not familiar with the term "shoe".
I assumed that it is the plastic end pieces that are attached to the rod assembly.
If so measured both sides and included photos One side is 1 inch, the other 1.25 inch.

Yes, Frank you are right. The term "shoe" refers to the nylon attachments on both ends of your balance. To properly measure the depth of your bottom balance shoe, look at the diagram below. Once you have measured your shoe, you will have a better idea on which balance to go with as a replacement.
Based on your measurement, it sounds like you will go with the Series 380 channel balance. Give it another look to confirm this is the replacement balance for you. Let us know if we can be of any further help.
This assembly will replace the one on the left side of the window when looking towards the window. Frank
One difference, the balance in your picture has a hook while my non functioning balance does not. Perhaps that is the reason it does not work. I enclosed pictures which show my balance without the hook. Thanks Frank
If your balance lost its hook that would make sense for why it doesn't work anymore. It happens all the time.
