Need Balance
Thanks for reaching out! I am confident that we can supply you with the custom balance assembly you need, however, it will need to be a custom assembly. You have the Series 770 Channel balance with a 62-105 Heavy Duty Pointed Hook attached.
To get you set up with an order link for the correct balances, I'll need to know the length of the metal channel as well as the stamp from the back of it. The stamp from your balance is hard to make out in the photos you've provided.
I'll also need to know the exact number of balances you intend to purchase. There is an order minimum of (2) balances per custom balance.

The shoe you have appears to be our 15-208 Pivot Lock Shoe. This is compatible with the balance you have installed in the window.
To confirm that this is the right replacement, you'll need to remove it and carefully compare the measurements. I am confident this is the replacement you'll need.
