Thank you for waiting during this busy season. The top pivot you show on the header has two installation holes. That leads us to believe you may need the 79-097 pivot set used for US Aluminum and Tubelite brand commercial doors. Review the specifications for this part along with the related video to see if it matches your repair needs.
