I can't be sure without seeing the entire operator and how it attaches to the sash frame but the 39-315 would be a good place to start depending on how the specifications of the split arms compare to yours. Note that the shape of the base differs form your old style assembly, which could interfere with the sash frame are require modifications. Take a look at the specifications below and related parts to see if this might work for your repair. Thanks for posting!
