It appears you have the Series 795 channel balance which comes with the 15-195 closed-cam shoe by default. However, yours has an open cam like our 15-195B. You'll need to add this part to your cart separately and then attach it to the balance once you receive your order. The related video will demonstrate how to do this, it's very easy! 

The stamp on your balance's metal channel doesn't match our strength options so you'll need to weigh your sash to determine the correct one. There's a chart under the product description on the S795 store page that you can refer to. 

One important detail to note is that the Series 795 attaches to the top of the track using a screw, not a hook. If yours needs a hook then let me know; we can still help but your balance will have to be custom made.
