Need help to identify and getting the parts for my windows

Question

Sticker on my windows say:

3150 White Single Hung Sloped EGRESS Window
6/6 Grills White
36 1/2 X 59 1/2
4235 Sl2627.18.D09 128241 6/6 Grills

I measured the length of the spiral balance
Tube length = 28 inches
Diameter = 5/8
Nylon Bearing Color = Red

I am not sure about Pivot Shoes since they are broken. I see blue color lock in the pivot lock shoe.

I need 12 spiral balances and 12 Pivot lock shoes. Pls. help me
