As you may see from the third picture, the imprint is: 26 4 CO Caldwell. I believe I need a 350 series channel, but I am uncertain of the weight. Does the 26 4 correspond with the 2640 option or?
The hook is also unusual in that it is a flat hook with a "C" stamped on it.

I do believe the Series 350 is the correct replacement for your balances but I'll need to see a photo of the spring to confirm. I'm particularly looking to see a photo of the bracket connecting the spring and the cord (string) in the middle/center of the balance.
Also, may I see a photo of the hook you are refering to?
Thanks for the additional photos. The S350-27 should work fine to replace the balance you have here. The 26 4 stamp can likely be replaced with the 2640 stamp option, which will support a sash weighing between 19 to 26lbs.
You'll need to physically weigh the sash to confirm whether it falls between that weight range.
