Closest I can find is Product Code: 39-036Truth Hardware Entrygard Dyad Window Operator
but not sure that's right. Please help.

The 39-036 indeed looks like a match. Your operator appears to be an older version. If you haven't already, you'll want to compare the specifications to make sure they align with your old assembly.
