Hi Tom. See our S765-27. Go over all the specifications and images listed on the product page to confirm this matches your old hardware. Your balance's stamp code suggests you need a stamp option of 26-5, which is for a window sash in the weight range of 17 through 23 lbs. Let us know if you have any questions.
