Need replacement handle for sliding patio door

Hi and thanks for posting. Check out our 82-080; like your handle, the installation holes are on the side of the base. From their centers the distance between these holes is 4-15/16". Take a look and see how this matches up with your piece. 

I'm not sure I understand. Are you referring to the mortise lock? If so, this is a separate part. It isn't something built into the handle itself. The 82-080 is compatible with a couple different kinds, but yours specifically looks like our 82-268B. I'm not sure if you need to replace this or not, since the damage seems to be localized entirely on the handle itself. Your mortise lock is probably functioning just fine.
