pLEASE PROVIDE PART NUMBERS & COST FOR BALLANCE, LEFT AND RIGHT PIVOT BARS, AND THE LOCKING CAM THAT CONNECTS THE BALLANCE TO THE PIVOT BARS.

Hello there!
Thanks for reaching out to us. We're happy to help with this. This looks similar to the Series 770 but with the 62-105 hook so that it can connect to a pivot shoe like our 15-105. This configuration isn't a stock balance of ours but we can provide this for you via custom order. I'll just need additional information to get this moving for you.
- Can you confirm the length of the metal channel for the balance?
- Can you confirm the precise weight of the sash?
- How many balances are you looking to order?
- How does your balance install at the top of the window, with a screw or some kind of hook? May we see a photo of it?
Your pivot bar is discontinued but you might be able to use the 26-241 as a possible substitute. Take a look at the specifications and see what you think.
