The stamped number you provided only indicates the spring strength of the balance and not in the identification of the balance. We can definitely offer a replacement for you, we just need some more information. We would like to see photos of the entire balance removed from the window as well measurements and dimensions of the balance.

It's hard to tell from the photo... does the bottom attachment have the same square wings as the top? If not, you have a Series 395 balance, specifically S395-37. However, you still need to choose an option. The option on your old balance was 36-9, but that option no longer exists. Your new option will begin with 36, but it will be followed by some different number – a multiple of ten: 10, 20, 30...

There is a chart in the product description that tells you which option to use for a 37 pound sash. For S395-37, it will be 3660.
