This sliding door handle set is a match for the 82-086 Legacy Keyed Handle Set. The mortise lock appears to be missing its faceplate but the 82-265 Multi-Point Mortise Lock is likely to be the correct replacement. You'll need to measure the distance between the installation holes, on the door, to determine if this is the correct match. 

Also, keep in mind you may also need the 82-249 keeper, as there is a chance the existing one may not be compatible with this mortise lock.
