I am looking for the part that connects the sash to the metal rod in each side of the window frame. The part hooks on to the metal rod and has a slot for the small rods that stick out of the lower sides of the sash.

Looks like you need a shoe similar to our 15-195, but this won't work for you. The problem is that big "dent" they designed at the bottom of your metal channel. Because of this, you will need an all new Series 795 channel balance. I apologize for the inconvenience.
