Thanks for your patience. Your pivot shoe looks a lot like our 9/16" Thick pivot shoe, part 15-129. Take a look over the dimensions below and compare them to your existing hardware to confirm that this is the right fit.

We do not carry an exact replacement for your pivot bar, however our 26-424 is worth considering as a replacement. Again, look over the dimensions to confirm that this will be a fit. So long as the installation hole matches up, this should work as a suitable replacement.
