It looks like you have the Series 795 channel balance with the 15-195B open cam shoe (sold separately) but you'll need to compare our dimensions to yours to confirm. There should be a stamp on your metal channel that indicates what stamp option you'll need. If there's not, you'll need to weigh your sash on a scale and refer to the chart on the S795's store page to determine which option you'll need based on the weight of your sash. Keep in mind, when determining length, only measure the metal channel and do not include the fittings in your measurement. 

One important detail to note is that the Series 795 attaches to the top of the track using a screw, not a hook. If yours needs a hook then let me know; we can still help but your balance will have to be custom made.
