Ply Gem window block and tackle balance

It appears you have our Series 795 channel balance. This comes with the 15-195 closed cam T-lock shoe by default so I recommend looking over the specifications and comparing them to your old shoe to confirm this matches what you have. There should also be a stamp on the metal channel that will tell you which spring strength option you'll need to choose. 

One important detail to note is that the Series 795 attaches to the top of the track using a screw, not a hook. If yours needs a hook then let me know; we can still help but your balance will have to be custom made.
