We took out the window and it weighs 41 pounds.

Thanks for reaching out to us! Your sash weight exceeds our stamp options so this must be a custom order. However, the pivot bar is a bit odd and I'm not immediately familiar with this style. Please allow us a bit more time to look into an alternative, we will reach back out as soon as we have more information.
As for the pivot shoe, the 15-129 may be what you're searching for but I recommend you compare the specifications below to confirm.
