This looks like a Series 795 Channel Balance with a 15-195B T-Lock Shoe, sold separately. For the Series 795, measure the length of the metal channel to determine the length. Your old stamp code is a little unusual, so we advise weighing the window sash. You can then match that exact weight with that weight range listed alongside the stamp options on any given balance's shop page. The video below shows how to swap out the 15-195B with the shoe that comes installed by default to these balances (the 15-195).
