This old rod balance isn't made anymore, but you should be able to replace it with our Series 770 Plain Tilt-Window Channel Balance. You'll have to measure the length of the rod to determine the length channel balance you need, and then obtain the exact weight of the window sash to know what stamp option you require. For your pivot bars, see our 26-021. As far as your pivot shoes go, take a look at our 15-129 and 15-135. Compare the width and thickness dimensions with your old shoe to see which matches best.
