Hi Bill. Check out our S380-35 or S385-35 Channel Balances. The only difference between these two balances is the 'depth' of the bottom nylon shoe. Compare both with your old hardware and see what matches best. You will need a stamp option of 3440.

If your old Top Sash Guide has that general style though, you'll ultimately just have to go over all the dimensions and see which matches best.
