For the top sash guide and take-out clip, I recommend our 18-171 and 16-002. We can help with the channel balance but it will have to be a custom order due to the configuration. The pulley wheel is built into the bottom fitting itself and this balance features an extended cord travel as well as a metal channel length ending in 5/8" which is what makes it custom. For example, your metal channel should measure 27-5/8" long. Custom orders are nonrefundable so you may want to double-check to be safe. 

In order for me to get this moving for you, we will need more information. Can you remove an undamaged balance from the window and show us clear photos of the top and bottom fittings? We will also need to see a photo of the metal clip attached at the end of the pulley cord.
