Replacement window parts to be identified

These lifts are on my Pella vinyl tilt window.
The sash weighs 39 pounds.
I would like to replace both balances , Slide blocks and metal pivots on sash could you please help me with the Part numbers

Hello,

We can supply the S795-28 (28G) balances with the 15-198 hook attached but it will need to be a custom order. We'll just need a bit of time to get a price quote together for you. Also, please note that the 15-194 T-lock shoe would need to be purchased separately. See our video below showing how to swap out the stock shoes that come with these balances. Thank you!
