S795 with Hook

Your balances resemble the S795-24 and S795-28 except, as you mentioned, it looks like yours uses the 15-198 hook at the end of the pulley cord for installation. If so, this would need to be a custom order. 

I would recommend removing your t-lock shoe from the window to confirm if the width matches the 1-1/4" wide 15-195 shoe that comes with these balances.
