I need a replacement for this truth casement arm. It seems to have a 5 digit code on it but I'm very unsure of the first 2 numbers. They might be 91, but they're quite rusty and hard to make out. The last 3 numbers appear to be 605

Hello there. It looks like you need new casement hinges. I believe our 36-004-10 10" Hinge Set is likely what you need. Review the dimensions below to confirm. Let me know if anything differs.

The crank is popping and not closing properly as well. Is there a crank that goes with that, or do you need more info?

The 36-004-10 is compatible with a ton of different operators, so there isn't any one pairing. That said, looking at your photos, the design of your original operators closely resembles our 39-316. This may be what you want but you should once again review our dimensions, just in case.
