Hello! I recommend you have a look at the 79-097 with the two mounting holes in the top frame pivot and the 79-133 with the three holes in the top frame pivot. Looking at your photos it appears that it could be one of these. I recommend you look over the dimensions and hole locations to see how they compare with your old hardware.

Both the top and the bottom hinges have just two holes for screws. See pics.

I do believe the 79-097 is the pivot you are looking for. These pivots are used on US Aluminum commercial doors. I recommend seeing if you have any name tags on the door stating US Aluminum.
