Your balance is similar to the S751-34, but the knife lock shoe attached to the pulley looks a bit different. We'll need to see the shoe fully removed from the window jamb and have you write out the width and thickness to determine the best replacement option. I'd also suggest weighing the window sash to confirm the spring strength needed for your balances. 

As for the corn pivots, we don't carry exact replacements for your originals, but the 26-150 pair is commonly used with the S751 balances. Review the specifications below and see what you think.
