Thanks for reaching out to us! The shoe you have here is the 15-177 Coil Balance Shoe. I am confident this is the replacement, you'll but be sure to double check the dimensions to confirm.
This shoe is compatible with the Series 152 Coil Balance, secured to the shoe using the 33-072 screw. To determine which spring(s) you need for your assemblies, you'll need to check for any stamps/markings on the metal coil. If there aren't any, you'll need to confirm the exact weight of the sash and divide that by the total number of coils in the window. This will give you the spring you need from the list of Series 152 coils.
The cover can be replaced with the 15-143 Coil Bushing, 15-144 Coil Bushing Cap, and the 15-145 Coil Support Barrel.
I'm happy to identify the replacement needed for your pivot bar, but I will need to see a photo of an undamaged pivot bar removed from the bottom of the sash to confirm the correct replacement.
